NGN Case Q: The 49 year old client experiences  polyuria, Polydipsia, thirsty and nocturia. She needs to urinate often, both day and night. She had a major head injury 1 year ago. She has muscle pain and weakness now. She is difficulty performing simple mental tasks. Regarding these clinical  symptoms, which nursing Interventions are indicated or not indicated  for the patient?


1-Monitor vitals & neurological status.

2-Monitor intake and output

3-Monitor specifc gravity of urine for excessive urinary output.

4-Monitor for signs of increased intracranial pressure.

5-Implement seizure precautions.
